I agree i have updated my consultation form and notified my therapist of any allergies or medical conditions allowing my therapist to carry out my treatment in a safe manner.
I also am aware of the cancellation policy and agree if cancelled within 48 hours of my appointment sceduled i will be charged for my full appointment.
